This form is used to submit a request for accommodations if you (the student) may be absent from class(es) due to a regularly occurring faith, belief, and/or religious observance.
There are several ways students can request a faith, belief, and/or religious observance accommodation. Some options are listed below, and the university supports you to use the option that best fits your needs. If you choose to submit the Faith, Belief, and/or Religious Accommodation form, it will be reviewed by the Vice President for Student Affairs office and you will be included in all communication with your instructor(s).
- You may discuss your accommodation needs with your instructor. It is possible they will support your request without needing the Faith, Belief, and/or Religious Accommodation form.
- After talking with your instructor(s), they may ask you to document your request with the Faith, Belief, and/or Religious Accommodation form.
- Some students will prefer to submit their request using the Faith, Belief, and/or Religious Accommodation form without talking with their instructor(s).
- If you are uncomfortable or unsure how to approach your instructor, you can submit the Faith, Belief, and/or Religious Accommodation form.
To allow time for processing and for faculty to accommodate your request, please submit this form at least one full week (7 days) prior to your anticipated absence. Requests submitted fewer than 7 days prior to the date of a faith, belief, and/or religious observance may not be accommodated.
