The Division of Student Affairs contract with Anthology (formerly CampusLabs) expired on June 30, 2025 and we have transitioned to full use of Qualtrics via the university enterprise license.  This means all faculty, staff, and students have access to create an account at no cost to you to create and administer surveys in your role at CSU.

To access the login, go to https://colostate.qualtrics.com and use your NetID university credentials as your login. It should take you to the Qualtrics homepage, from which you can create surveys and collaborate with others in the platform.

Once you have accessed the platform, please complete the Microsoft Form to be added to the “division” in Qualtrics.  This allows for easier sharing of resources and collaboration within groups.
